Citywide Immunization Registry (CIR) - NYC Health (1)

The New York Citywide Immunization Registry (CIR) keeps immunization records for all children and adults who live in the city. CIR consolidates immunization information and shares it with health care providers, families and agencies concerned with public health.

You can register your practice online. Visit CIR Access for detailed instructions if you are an Immunizing Provider (PDF) or Non-immunizing Provider (PDF).


Update your practice’s registration information using the CIR Facility Manager, then navigate to the CIR Facility Lookup tab. Do not re-register.

If you think you may be registered and have forgotten your facility code, enter your information by logging in to the Facility Manager and then navigate to the CIR Facility Lookup tab. Do not re-register.

NEW: Single Sign-on solution instructions. Newly registering and current CIR registered participants will register their individual work email to create a Single-Sign-on account through the CIR Authentication Server application. The Single-Sign-on account allows access to CIR applications, including the Facility Manager and Online Registry. Current Online Registry users will be directed to register with the CIR Authentication application.

Starting on October 3, 2023 at 1 p.m., the Online Registry log-in screen will be replaced with the CIR Authentication Server log-in screen. At that time you will no longer use your Online Registry User ID and Online Registry password. Instead, the CIR Authentication Server tool will prompt you to create a single sign-on account to access the CIR Online Registry and the CIR Facility Manager application. The account will be tied to your individual work email address and a new password that you will create. Current Online Registry users will be directed to register with the CIR Authentication Server tool.


Reporting Immunizations Requirements

NYC health care providers report immunizations to the CIR as mandated by New York State Public Health Law and the NYC Health Code. Pediatric providers are required to report all immunizations administered to children aged 0 to 18 years. Vaccines administered to adults 19 years and older should be reported to the CIR with documented verbal or written patient consent.

To enable monitoring of equitable vaccine distribution, it is essential that vaccination providers gather and report accurate data on vaccine recipients’ characteristics, including race and ethnicity. Vaccine providers are encouraged to consult best practices for collecting and reporting race and ethnicity to the CIR.

The use of the CIR to verify the vaccination status of employees is not authorized under New York State law or the New York City Health Code. Employers with vaccination requirements must ask employees to provide documentation of vaccination.

New Yorkers may obtain their vaccination records from the CIR online or from their vaccination provider.
